I know everybody’s least favorite thing to do is paperwork. But it is essential to keeping your children safe while they participate in the many wonderful events we offer.

Below are some of the basic forms you will need for many CISV events.

NATIONAL OR LOCAL MINI-CAMPS:

Youth legal form – domestic

Covid waiver

health form: use the same as for international programs, but you only need to fill out sections A,B and D

 

CHAPERONES/ ADULT VOLUNTEERS CLEARANCES

 

INTERNATIONAL PROGRAMS:
